Trans-sectoral needs-based treatment of patients with heart failure and development of an alternative remuneration model

For many patients, the diagnosis of heart failure is associated with multiple and exhausting stays in hospitals, which cause high costs for the health care system. The aim of the sekTOR-HF project is to provide cross-sectoral health care services for patients with heart failure. The project aims to reduce avoidable hospitalization by providing suitable services while using telemedicine. Furthermore the project develops an alternative reimbursement concept. The RWI evaluates the health status of patients over time.
