sekTOR-HF – Transsektorale bedarfsorientierte Versorgung von Patienten mit Herzinsuffizienz und Schaffung eines regionalen Vergütungsmodells

As part of the sekTOR-HF innovation fund project, the report examines a new telemedicine-based care model for patients with chronic heart failure. The aim is to assess the impact on hospital stays, healthcare costs, quality of life, and self-care. The evaluation is carried out using a quasi-experimental difference-in-differences approach and focus groups with patients and medical staff. Results show that hospital stays and healthcare costs decreased, the latter by about 50%. However, the intervention does not significantly affect quality of life or self-care. The telemedicine concept is rated positively by the majority. However, due to the small sample size and possible differences between the intervention and control groups, the results are only of limited reliability.
